The Care Coordination Center Helps Seniors Recover After Hospitalization and Chronic Illness

What is the Care Coordination Center (CCC)?

A center for specialized care of patients with complex medical problems. It provides patients going home from the hospital with a 45 to 60 minute visit to go over discharge instructions with a doctor or nurse practitioner.

Why go to the CCC after being discharged from the hospital?

Studies have shown that people do not remember all of the instructions given to them during hospital discharge. This can affect your ability to recover fully and lead to further hospitalization.

What happens during my visit?

You will be seen by Dr. Tauseef Haider and/or a nurse practitioner, and a nutritionist and a pharmacist. Together these experts will assess your medical conditions, test results, medications and answer any questions. They will help coordinate and schedule your follow up visits to specialists and physicians. Some patients may receive a referral for THIPA Care Management, Torrance Home Health Services or hospice.

What about my Primary Care Doctor?

Your primary care doctor will receive a detailed report of your visit. The CCC team eventually transitions your care back to your primary care physician.

How do I make an appointment?

The CCC is by referral only. Your hospitalist, primary care physician, oncologist or another specialist can refer you. You can also call the CCC with questions at 310-784-8713.
